For more than 15 years, the Foundation has awarded grants to support projects focusing on chronic disease, family health, and service delivery systems. These grants have been used to establish strong community impact, demonstrate new and sustainable ways to solve health problems, illustrate the effectiveness of early intervention and preventive health programs, and improve and ensure access to quality health care for underserved populations in both urban and rural areas. We are proud to support organizations that work hard to improve the health of the communities they serve.
